This is not the office's Notice of Privacy Practices
The medical information the office handles as part of your care is governed by the practice's Notice of Privacy Practices, a separate document required under federal health privacy law (HIPAA), with its own effective date. That notice isn't replaced by this page. You can request a copy directly at the office.
Information you send us
The "Request an appointment" page does not yet accept online requests. For now, the way to book is by calling our office at (408) 258-3200. Before we turn on online submission, it will go to an approved system with proper access controls and privacy protections for patient information, not the office's public email address.
Information collected automatically
This site currently has no analytics tool installed (for example, Google Analytics). If one is added later, it will be used only in aggregate to understand general site use (page views, clicks on call and appointment buttons, browser type), will never include medical information, and will not load on the appointment request page or its confirmation. This policy will be updated before that happens.
Sharing
We do not sell your information. Form submissions are delivered to office staff to follow up on your request.
Third-party map and content
The contact page includes a Google map. Loading it may allow Google to collect data under its own policies.
